Health coaching helps those who have behavioral or chronic medical conditions. Coaching is available to adult State Health Plan members at no cost. You can learn how to manage your symptoms, and delay or even prevent many of the complications of these diseases.
BlueCross BlueShield of South Carolina (BlueCross) identifies participants by reviewing medical, pharmacy and laboratory claims. You are automatically enrolled if you are someone who could benefit from one of the programs. However, the programs are voluntary, and you can opt out at any time. You may also self-enroll in one or more of the available health coaching programs.

Behavioral health
A health coach will work one-on-one with you and offer you support with the following categories. Your coach will encourage you to follow your treatment plan, help you set goals and teach you how to handle symptoms.
- Addiction recovery;
- Attention deficit hyperactivity disorder (ADHD);
- Bipolar disorder; and
- Depression.

Chronic conditions
Managing a chronic condition, like the ones below, can be difficult. A health coach can help you better understand your condition and how to manage it. The health coach will also work with your physician to develop a plan to take charge of your illness.
- Asthma (for adults and children);
- Chronic obstructive pulmonary disease (COPD);
- Congestive heart failure;
- Coronary artery disease;
- Diabetes;
- High cholesterol;
- Hypertension (high blood pressure); and
- Migraine.

Healthy lifestyles
If you are ready to get on track with your health but aren’t sure where to start, a health coach can help. Your coach will help you create a personalized action plan for meeting your goals.
- Back health;
- Metabolic health;
- Stress management; and
- Weight management for adults and children.

Maternity
Coming Attractions supports mothers throughout their pregnancy and postpartum care. It also assists with Neonatal Intensive Care Unit infants or other babies with special needs until they are one year old. You will connect with a maternity care nurse and receive educational materials through your pregnancy and your baby’s first year of life. You also learn how to receive a breast pump at no cost. We encourage mothers to enroll during their first trimester.
