How do subscribers make changes

The easiest way for subscribers to make coverage selections during open enrollment is through MyBenefits (mybenefits.sc.gov).

Employees who wish to apply for or apply to increase their Optional Life or Dependent Life-Spouse coverage, or those who wish to make changes to their Supplemental Long Term Disability coverage that require medical evidence, cannot make their open enrollment changes through MyBenefits. Instead, they must complete a Notice of Election (NOE) form, sign it and return it to their benefits administrator by October 31, 2019. Benefits administrators must submit the NOE to PEBA once they receive approval for coverage from MetLife or The Standard.

Optional Life and Dependent-Life Spouse changes

Employees who wish to apply for or apply to increase their Optional Life coverage can complete a Supplemental Form that includes five health questions. If they answer yes to any of the questions they must complete a full Statement of Health form. Employees who wish to apply for or apply to increase their Dependent Life-Spouse coverage must complete a full Statement of Health form.

Each employer must provide its custom Supplemental Form and
Statement of Health to its employees. Learn more about the life
insurance process during open enrollment here.
